If you are looking for a way to remove your email address from the study's email list, you will need to complete the opt out form.
You can also opt out by calling +44 20 3108 4358.

As discussed in the PIS, you may need to contact your Trust's Patient Advice and Liaison Service (PALS), you can find their information here. There is also a link for finding out more if you would like to opt out of your Trust using your patient data for research. You can also connect with the study coordinator if you have any questions or concerns.
